How to Provide Intake Forms to Participants
There are two ways to offer automated intake forms to clients: send them from an existing case or embedded on your website.
Send an intake form inside a case
Here's a scenario: A potential client calls wanting to work with you.
Step 1: Add a new case in ADR Notable. TIP: You only need a case title and an assigned firm to create a case (as well as any required custom fields). No party information is required right now - but we do recommend collecting the email address(es) from the potential client(s) for later.
Step 2: Save the case. You'll see the blank party information on the case dashboard. With Automated Intake, you don't need to gather and manually input that information - you can leave that to the client by send a form directly inside the case. Here's how:
In the party profile, click the Send Intake Form icon:

Input the email address(es) of the party(s) you'd like to invite to fill out their information in the case. Choose the form and hit Send.

The party(s) will receive an email with a link to the form you chose. The client experience will differ based on whether the form is Authenticated or Unauthenticated (learn about the differences in our form types here).
Once a party has completed and submitted a form, you will be notified (learn more about smart notifications) and the case information will auto-fill in the case.
Any and all party details outside of the general info such as name, communication, email, address, etc., will be stored in the Party Additional Details sections in the party profile.
You can type more information into the response fields on top of what the party submitted (if you want to expand upon their original answers as you learn more about the case).


Embed a form on your website
Each form will have its own unique URL that can be hyperlinked or embedded on your website. This style of auto intake will create a new case in ADR Notable for you.
With each individual form, you can choose how it will be titled (which can be changed/edited) and what case type it will be assigned. For example, you can have a form that will assign it as a civil case and another form for community cases.

You will be notified when a case is created via the URL on your website.
