How To Embed an Automated Intake Form on your Website

Give clients the ability to create a case directly from your website. This article will tell you how to insert the intake form to your web page. 

Navigate to Firm Settings and select Intake Forms.

Select the form you wish to add to your website and click "Copy Form Link".

One the backend of your website, you can either hyperlink the URL to a button/text OR paste the URL into your site: 

If you have a webmaster, provide them with this link and instructions regarding where to place it. If you manage your site, simply place this URL where desired.

If you would like to preview the form, click preview:

Completion of this Intake Form will create a new case in your ADR Notable platform.
